Pain Relief

6 covered treatments for chronic back pain most seniors never use.

The short answer
  • Chronic back pain often gets treated with a shrug and a refill, while covered treatment goes unused.
  • Medicare has covered acupuncture for chronic low back pain since 2020: up to 12 sessions a year, with more if it's helping.
  • Physical therapy, behavioral pain care, and monthly care management are covered too. Almost nobody is told.
  • A Careway Advocate finds every covered option, gets the referrals, and books the visits.

Careway Advocates are dedicated health experts who unlock the Medicare and community benefits most seniors miss. Your Advocate is covered by Medicare.

What we handle for you

The 6 covered options for chronic back pain.

01

Acupuncture — covered since 2020, used by almost no one.

Medicare covers up to 12 acupuncture sessions in 90 days for chronic low back pain, and adds 8 more if you're improving, up to 20 a year. It's real, evidence-backed relief that most people still believe Medicare would never pay for. The catch: it must be chronic low back pain, and the provider has to meet Medicare's requirements.

How Careway handles it
Your Advocate confirms you qualify, finds a provider who meets Medicare's rules, books the sessions, and tracks your visit count so covered relief doesn't go unused.
02

Physical therapy that treats the cause, not just the symptom.

Medicare covers physical therapy when it's medically necessary, and for back pain that means core strength, flexibility, and posture work that actually changes how the back feels month to month. Many seniors quit after one round; ongoing covered care is often available when it's still needed.

How Careway handles it
Your Advocate gets the referral, finds an in-network therapist near you, books around your schedule, and arranges transportation if getting there is the barrier.
03

Behavioral pain care, covered like any medical visit.

Chronic pain wears on sleep, mood, and patience, and pain and stress feed each other. Medicare now covers therapy and counseling the same way it covers medical visits, including approaches built specifically for living with long-term pain. Few people are ever offered it.

How Careway handles it
Your Advocate finds covered providers, including telehealth options you can do from home, and handles the referrals and scheduling.

04

Monthly care management for chronic conditions.

If you live with two or more chronic conditions, Medicare covers chronic care management: regular check-ins, a written care plan, and someone tracking whether the plan is working. For pain, that means the difference between "come back in six months" and a plan that adjusts when something isn't helping.

How Careway handles it
Your Advocate keeps the whole pain plan moving between appointments: tracking what's helping, flagging what isn't, and making sure nothing waits half a year to get adjusted.
05

The right specialists, found and booked for you.

Good back care often means a pain specialist, a physiatrist, or an orthopedist, and finding one who takes your plan, has openings, and is close enough to reach is a project in itself. Many seniors give up at the search stage and just live with it.

How Careway handles it
Your Advocate finds in-network specialists with real availability, books the appointments, arranges the rides, and follows up so referrals and imaging orders don't fall through the cracks.
06

A dedicated Careway Advocate — the benefit that books the other five.

Every treatment above is covered and real. The reason they go unused is that each one requires knowing it exists, qualifying for it, finding the right provider, and pushing the referral through. Your Careway Advocate does all of that, and the Advocate itself is a covered Medicare benefit. One sign-up, every option on this page.

How Careway handles it
Confirming your eligibility takes 2 minutes. From there, your Advocate finds every covered treatment you qualify for and books it, so relief stops depending on you doing the research.

The details, if you want them

Why back pain goes under-treated after 65.

Chronic low back pain is one of the most common reasons older adults lose mobility and independence, and one of the least systematically treated.

  • It gets normalized. "That's just age" ends more conversations than it should. Pain that limits your life is a medical problem with covered treatments, at any age.
  • Visits are short. A 15-minute appointment has room for a refill, not for building a multi-part plan across acupuncture, therapy, and specialists.
  • The good options take legwork. Each covered treatment needs a referral, an eligible provider, and scheduling. That work lands on the person in pain.
  • Nobody follows up. Without someone tracking the plan, a treatment that isn't working just continues, and one that would work never starts.

That's the gap an advocate fills: not new medicine, but making the covered medicine actually happen.

What Medicare covers for chronic low back pain.

Acupuncture
Up to 12 sessions in 90 days for chronic low back pain lasting 12 weeks or more, plus 8 additional sessions if you're improving. Maximum 20 per year.
Physical therapy
Covered when medically necessary, in outpatient clinics or at home through home health when you qualify.
Behavioral health
Therapy and counseling covered like medical care, including telehealth from your living room.
Chronic care management
Monthly coordination and a written care plan for people with two or more chronic conditions.
Specialist care
Pain medicine, physiatry, and orthopedics visits covered under Part B, with imaging when your doctor orders it.

One honest note: Medicare's acupuncture benefit is specific to chronic low back pain. Pain elsewhere has covered paths too, but they run through the other options on this page.

Why these treatments get missed.

The coverage exists. The reason people don't use it isn't that doctors are hiding it. The system isn't built to find you; it's built to be applied to.

  • The acupuncture benefit is new enough that many patients, and some providers, still don't know it exists.
  • Each treatment has its own qualifying rules, provider requirements, and referral path.
  • Living with pain drains exactly the energy that chasing referrals requires.
  • When nothing is tracked between visits, "try this and come back" becomes the whole plan.

Your Careway Advocate's job is to run the plan for you, start to finish, and to keep adjusting it until something works.

"I'd been told to stretch and take something for it for years. My advocate got me covered acupuncture and a real physical therapist. It's the first time anyone treated my back like a problem we could actually work on."

Frequently asked questions

Yes. Careway operates as a Medicare-billable service. Most patients pay little to nothing out of pocket. Your eligibility check tells you exactly what you'd owe before you commit.

No. Your doctors stay your doctors. Your Advocate works alongside them, handling referrals, scheduling, and follow-through, and looping them in on what's helping.

No. Careway physicians and Advocates don't prescribe or change medications. They complement your own doctors, unlocking covered treatments and keeping the overall plan coordinated.

Medicare added the benefit in 2020 after reviewing the clinical evidence for chronic low back pain, and covered providers must meet training and licensing requirements. Like any treatment, it's a conversation with your doctor, which your Advocate can help set up.

Yes. With your permission, your Advocate communicates with your family (sons, daughters, spouses) so everyone stays on the same page. That's especially useful when family lives in another state.

The eligibility check takes 2 minutes. From there, you usually have your first call with a Careway physician within a week, and an Advocate assigned the same day.

The bottom line.

Chronic back pain narrows life a little at a time: shorter walks, skipped plans, worse sleep. Medicare covers real treatment, from acupuncture to physical therapy to behavioral pain care, and most of it sits unused because nobody's job is to set it up.

Let your Careway Advocate make it their job, and find out what actually helps.
