What an advocate actually does for a member

A member left the hospital on Friday with a discharge packet and no follow-up appointment. Their advocate books the visit, moves the discharge summary to the primary care office, and reconciles the new prescriptions against what is at home.
A prior authorization has been sitting for nine days. The advocate calls the plan, finds the request is missing a chart note, gets the ordering office to send it, and sets a reminder to check back in forty-eight hours.
A monthly check-in surfaces that a member stopped a heart medication because of the copay. The advocate tells the prescriber cost is the barrier, screens the member for Extra Help, and compares pharmacy pricing on the alternative.
A member has missed two dialysis sessions because their ride fell through. The advocate arranges recurring county transportation, sets up a backup plan, and lets the dialysis center know both are in place.
An out-of-state daughter calls worried her father sounds confused. With his permission, the advocate writes up what the family has noticed in plain language, sends it to his physician, and books the appointment.
